Qu'est-ce que la chirurgie bariatrique révisionnelle ?

La chirurgie bariatrique révisionnelle ou parfois connue sous le nom de chirurgie de révision, fait référence à une chirurgie de perte de poids qui corrige ou améliore une chirurgie de perte de poids antérieure, telle qu'un anneau gastrique, un manchon gastrique, un pontage gastrique ou un mini pontage gastrique, qui n'a pas déclenché suffisamment de perte de poids ou eu des complications majeures. Même si c'est rare, il peut y avoir de nombreuses raisons pour lesquelles les chirurgies bariatriques peuvent ne pas déclencher une perte de poids suffisante, la page de chaque chirurgie contient plus d'informations sur les raisons pour lesquelles elle pourrait échouer.

Les chirurgies de perte de poids sont généralement très efficaces lorsque les patients suivent leurs restrictions alimentaires et font souvent de l'exercice, mais dans certains cas, la perte de poids peut tout simplement ne pas se produire. Comme chaque individu est unique, il arrive parfois que son corps ne réponde pas bien à un traitement particulier contre l'obésité.

La chirurgie bariatrique révisionnelle est recommandée si la perte de poids n'est pas optimale, si le poids est repris ou si des complications telles que des anneaux gastriques rompus ou déplacés, des problèmes de déglutition, un reflux gastro-œsophagien ou une poche gastrique agrandie sont présentes.

 

Comment fonctionne la chirurgie de révision ?

Toute chirurgie bariatrique peut être révisée dans toute autre chirurgie bariatrique, y compris elle-même, c'est-à-dire que le manchon gastrique peut être révisé en manchon gastrique.

 

Chaque chirurgie de révision peut être définie par la chirurgie qu'elle révise et la chirurgie vers laquelle elle se révise. La chirurgie en cours de révision pose une complexité plus élevée par rapport à la réalisation de la chirurgie sans qu'il s'agisse d'une révision, cela est dû au fait que le système digestif du patient a été modifié par rapport à la chirurgie bariatrique précédente.

 

La chirurgie de révision déclenche la perte de poids de la même manière que la chirurgie dans laquelle elle se révise, et la perte de poids attendue est également la même.

Comment se déroule la chirurgie de révision ?

La chirurgie de révision est une intervention chirurgicale plus difficile et nécessitant plus d'expertise que la première chirurgie. La première chirurgie peut être transformée en plusieurs chirurgies différentes avec une chirurgie de révision. Certaines des chirurgies de révision comprennent les suivants :

 

 

Révision de manchon gastrique à manchon gastrique

Resleeve peut être proposé lorsque le patient reprend du poids après une opération de sleeve gastrectomie. La perte de poids attendue du patient peut varier en fonction du poids repris après la sleeve gastrectomie. La révision de la sleeve gastrectomie est très efficace pour restaurer la structure de la sleeve en remodelant l'estomac, ce qui relancera la perte de poids.

 

Révision de l'anneau gastrique à d'autres chirurgies bariatriques

En raison de la simplicité chirurgicale de l'anneau et de son anatomie inchangée, les patients ayant besoin d'un anneau à la recherche d'une procédure de révision ont généralement de nombreuses options. Selon les objectifs, les circonstances de santé, les antécédents et l'anatomie du patient, celui-ci peut être en mesure de choisir entre une sleeve gastrectomie, un mini pontage gastrique ou un pontage gastrique de Roux en Y. Une fois l'anneau retiré, ces procédures sont effectuées comme elles le seraient si elles étaient effectuées en tant que procédure principale.

 

Manchon gastrique en révision du pontage gastrique

Le plus souvent, si la poche créée lors de la manche gastrique a été laissée trop grande, les patients auront une quantité insuffisante de restriction sur leurs capacités alimentaires et auront donc moins de perte de poids. Ou s'il y a un reflux acide sévère qui n'est pas suffisamment atténué par la médecine, la révision du pontage gastrique est une bonne option. Le pontage gastrique Roux En Y est appliqué au système digestif modifié, et le résultat est le même qu'un pontage gastrique non révisé. La chirurgie entraînera une perte de poids accrue et une réduction du reflux acide.

 

Pontage gastrique en révision du pontage gastrique

Il y a deux parties dans le pontage gastrique qui doivent être révisées : la poche gastrique et le réarrangement intestinal, qui est la partie malabsorptive de la chirurgie. Avant la révision du patient, une endoscopie haute, également connue sous le nom d'EGD, sera effectuée pour dimensionner la poche gastrique. Idéalement, le chirurgien essaie de réviser l'estomac en une poche beaucoup plus petite pour permettre une meilleure perte de poids. La boucle intestinale est également séparée et réunie avec le pontage, ce qui augmentera la longueur sautée de l'intestin grêle.

Comment se préparer à la chirurgie de révision ?

Les préparatifs pour la chirurgie de révision sont similaires à ceux des autres chirurgies bariatriques.

Le patient ne doit consommer aucun aliment ou liquide 12 heures avant la chirurgie afin de s'assurer que l'estomac est vide avant la chirurgie. Les restrictions alimentaires et médicinales suivantes doivent également être suivies avant la chirurgie :

  • L'utilisation de la vitamine K doit être arrêtée 30 jours avant l'intervention car il s'agit d'un anticoagulant, c'est-à-dire d'un fluidifiant sanguin, et qui augmente le taux de complications associées aux hémorragies internes.
  • Les contraceptifs oraux et tout autre contraceptif contenant de l'œstrogène ou de la progestérone, comme les stérilets, les patchs HRT ou les implants contraceptifs, doivent être arrêtés ou retirés 30 jours avant l'intervention car ils augmentent le taux de complications liées aux caillots sanguins. Les contraceptifs hormonaux et oraux peuvent être utilisés 30 jours après la chirurgie. Les implants non hormonaux sont sûrs et recommandés si vous envisagez un mini pontage gastrique.
  • Il faut éviter de fumer une semaine avant la chirurgie jusqu'à trois semaines après la chirurgie. Fumer provoque une constriction des veines et limite la circulation sanguine, ce qui peut entraîner des complications telles que des fuites.
  • La consommation d'alcool doit être évitée de préférence dans les 3 derniers mois précédant l'intervention. La consommation d'alcool une semaine avant la chirurgie doit être particulièrement évitée car elle augmentera la charge sur le foie et pourrait entraîner des complications avec l'anesthésie.
  • Le patient aura une consultation avec un spécialiste des maladies internes, un radiologue, un anesthésiste et un chirurgien général avant la chirurgie.
  • Les tests préopératoires à l'hôpital comprendront une échographie abdominale complète, une radiographie pulmonaire, un test de fonctionnement respiratoire, un électrocardiogramme, une enquête endoscopique de l'estomac et un test sanguin complet comme test et examens préopératoires.

 

When should Revision Surgery be applied?

Revision surgery may be needed if the patient cannot achieve adequate weight loss, or if the patient has regained weight as a result of the operation not being performed optimally.

Do diseases such as diabetes, asthma, and blood pressure prevent revision surgery?

Will there be weight gain again after revision surgery?

Revision Surgeries are successful for most patients with regards to permanent weight loss. However, in order to achieve this success, the patient needs to make lifestyle adjustments and change their diet. It should also be known that there is no guarantee of weight loss if these are not done.

Can you have a revision after a Gastric Bypass?

If the pouch between the stomach and the intestine is found to be widened, the “passageway” can be narrowed again endoscopically during the revision operation. The revisional operation also includes the separation of the intestinal loop and it being reunited with the bypass, which will shorten the effective digestive tract.

So my partner and I went for some treatments last week. David Turker is our coordinator. I was staying at Optimed and had Dr Ahmet for an arm lift. I also had one tooth implant at the dental clinic in the city. My partner has a hair transplant and consultation for new teeth. The hospital was immaculately clean and the nurses lovely and so helpful even though they mostly didn’t speak English. There was a translator there in situ 24/7. Nothing was too much trouble for the translators. We decided instead of staying in a hotel my partner would stay in the hospital with me. This meant some running about the city for the translators but they took it in their stride. I’ve had 8 operations in my 65 years in the UK with the NHS and this hospital and nurses were 4 times better in every way. They treated you with respect unlike here where they often treat you like a child! The dental clinic was out of this world in terms of cleanliness and the modern latest equipment plus the female surgeon who did my implant spoke English but the translator was there anyway. She was quick and effective and I could tell she knew exactly what she was doing. Compare this to me having 2 implants with a dental surgeon in Scotland. I paid £12,000 for 2. In Istanbul I paid £435 plus my service was second to none and no messing about. My partner had to have extra ozone treatment as he couldn’t stop smoking so it was affecting the blood circulation on his head. They gave him 3 treatments plus on the last night transferred us to the Lionel, a 5 star hotel nearer his clinic to facilitate his treatment before we went to the airport. We paid no extra for all of this. BookingSurgery. One of our translators came with us right up to the security part in the airport carrying our luggage. I felt that each step of the way I was in good hands. I run two businesses and have a governmental job which means I’m used to being in charge. Here I gave over complete control to others because I had complete trust in them. It was great to know they could instruct me and everything would be perfect. We gave our 8 translators $200 tip to share as I don’t think they get paid a lot but they were so lovely. We will both be returning for teeth work and I’ll probably have a face and neck lift with Dr Ahmet. I think the only reason the UK media spread negative reports of bad experiences in Turkey is because they shame the UK in terms of pricing and professionalism. So here it’s £6,000 per tooth implant and there it’s £435! They’re making a profit at that so imagine how we’re being ripped off in the UK paying thousands because we have no choice. The NHS won’t give us implants even though we’ve paid into the system since the age of 16. PS I didn’t have weighr loss surgery but lost the weight myself over several years. So just needed Booking Surgery to sort out saggy arms. So you can still have plastic surgery without having to lose stones and stones. So absolutely well done Booking Surgery. You’re fabulous.

It has been faultless from Booking the initial surgery which was a Gastric sleeve to leaving today. My surgeon Dr Katar has done an amazing job along with all his colleagues. Absolutely amazing! Booking surgery have literally changed my life! 🩷 Communication from the very start with my coordinator ( penny - also amazing btw 🩷 ) right to the very end was A***** I arrived at the airport, made my way to the meeting point and the driver was right there waiting for us bless him, provided water for the journey too! The hospital itself was so clean, so beautiful! I had an extended tummy tuck, breast uplift, muscle repair, 5 area of liposuction by Dr Nebil. This was all booked through my coordinator Robert, he’s been very thorough, anything I’ve asked he’s got the answer for me within 24 hours, he’s been a really helpful booking coordinator! I arrived at the airport and met the taxi man, he brought us to Yaşam Hospital were I met Martin the translator, he then took me for my tests, the next morning I met Dr Nebil were he told me all my tests were okay and he could draw me up for surgery, he advised me on what he thought was best but I didn’t want certain things and he didn’t push me for them just advised that to get the best results he’d recommend getting these done. 15 minutes later I was ready for surgery, got on the bed and down I went, I had sedation and then next minute (6 hours later) I woke up. I got took back to my room and was monitored closely, pain medication given and blankets to warm me up, 6 hours later I was allowed a drink, some food and took my first walk. The morning after surgery was when I got to see my results and OH WOW!!! Dr Nebil is a magician!!! I cannot get over my new body, I am beyond happy!!! Each morning my surgeon came to see me to check my incisions, and drains informed me on my blood results which were taken each morning. 4 days post op, my drains were removed, I felt the best I’ve felt all week. We leave tomorrow and will be picked up and taken to the airport. All the translators have been amazing, coming in to check I am okay and if we need anything, Oscar stood out the most to us he’s so cheeky and helpful he helped us with ordering some food and collecting it for us, but Martin and Stella have been great too! There were two other translators but I never got their names. The nurses have been fantastic! They come in 2 hourly to check my observations, they are all so friendly, and do their best to translate to you if the translator isn’t around. Steve the nurse is so gentle and friendly! There’s been many nurses who stood out to me but too many to name! My only negatives are that on day 2 and 3 I was left for 10-12 hours with no pain medications due to them not listening that my cannula wasn’t working, and also that I wasn’t due any medications but this hadn’t been explained to me. During my stay I did need 5 cannulas due to the care of them, they didn’t get flushed before every use and therefore I think they ended up closing and no longer worked. On some nights the nurses were rather loud, so this could be an issue for people if they struggle to get to sleep with noise. Honestly you hear all the horror stories about surgery abroad but this company has been amazing and I’d 110% recommend them to anybody contemplating surgery abroad!!
