¿Qué es la cirugía bariátrica revisional?

La cirugía bariátrica revisional o a veces conocida como cirugía de revisión, se refiere a una cirugía de pérdida de peso que corrige o mejora una cirugía de pérdida de peso anterior, como la Banda Gástrica, la Manga Gástrica, el Bypass Gástrico o el Mini Bypass Gástrico, que no desencadenó suficiente pérdida de peso o tuvo complicaciones importantes. A pesar de que es raro, podría haber muchas razones por las que las cirugías bariátricas no pueden desencadenar suficiente pérdida de peso, la página de cada cirugía contiene más información sobre por qué podría fallar.

Las cirugías de pérdida de peso suelen ser muy efectivas cuando los pacientes siguen sus restricciones dietéticas y, a menudo, hacen ejercicio, pero en algunos casos, la pérdida de peso puede simplemente no ocurrir. Se recomienda la cirugía bariátrica de revisión en caso de que la pérdida de peso no sea óptima, se recupere el peso o se presenten complicaciones como bandas gástricas rotas o desplazadas, problemas para tragar, enfermedad por reflujo gastroesofágico o bolsa gástrica agrandada.

 

¿Cómo funciona la cirugía de revisión?

Cualquier cirugía bariátrica se puede revisar en cualquier otra cirugía bariátrica, incluida ella misma, es decir, la manga gástrica se puede revisar en manga gástrica.

 

Cada cirugía de revisión se puede definir por la cirugía que está revisando y la cirugía en la que está revisando. La cirugía que se está revisando plantea una mayor complejidad en comparación con la realización de la cirugía sin que sea una revisión, esto se debe a que el sistema digestivo del paciente se modifica con respecto a la cirugía bariátrica anterior.

 

La cirugía de revisión desencadena la pérdida de peso de la misma manera que la cirugía en la que se revisa, y la pérdida de peso esperada también es la misma.

¿Cómo se realiza la cirugía de revisión?

La cirugía de revisión es una intervención quirúrgica que es más difícil y requiere más experiencia que la primera cirugía. La primera cirugía se puede transformar en muchas cirugías diferentes con la cirugía de revisión. Algunas de las cirugías de revisión incluyen:

 

 

Revisión de manga gástrica a manga gástrica

La manga se puede ofrecer cuando el paciente recupera el peso después de una cirugía de Gastrectomía en Manga. La pérdida de peso esperada del paciente puede variar dependiendo de cuánto peso se recuperó después de la Gastrectomía en Manga, la revisión de la Gastrectomía en Manga es muy efectiva para restaurar la estructura de la manga al remodelar el estómago, lo que reiniciará la pérdida de peso.

 

Revisión de la banda gástrica a otra cirugía bariátrica

Debido a la simplicidad quirúrgica de la banda y la anatomía inalterada, los pacientes de la banda que buscan un procedimiento de revisión generalmente tienen muchas opciones. Dependiendo de los objetivos del paciente, las circunstancias de salud, el historial y la anatomía, el paciente puede elegir entre someterse a una Gastrectomía en Manga, un Mini Bypass Gástrico o un Bypass Gástrico Roux en Y. Una vez que la banda se ha eliminado, estos procedimientos se realizan como lo harían si se hiciera como el procedimiento primario.

 

Revisión de manga gástrica a bypass gástrico

Más comúnmente, si la bolsa creada durante la manga gástrica se dejó demasiado grande, los pacientes tendrán una cantidad inadecuada de restricción en sus capacidades de alimentación y, por lo tanto, tendrán menos pérdida de peso. O si hay un reflujo ácido severo que no se alivia lo suficiente con medicamentos, la revisión en Bypass Gástrico es una buena opción. La derivación gástrica Roux En Y se aplica al sistema digestivo modificado, y el resultado es el mismo que una derivación gástrica no revisional. La cirugía resultará en una mayor pérdida de peso y un reflujo ácido reducido.

 

Revisión de bypass gástrico a bypass gástrico

Hay dos partes del bypass gástrico que se revisarán: la bolsa estomacal y el reordenamiento intestinal, que es la parte malabsortiva de la cirugía. Antes de la revisión del paciente, se realizará una endoscopia superior, también conocida como EGD, para dimensionar la bolsa estomacal. Idealmente, el cirujano trata de revisar el estómago a una bolsa mucho más pequeña para proporcionar una mejor pérdida de peso. El bucle intestinal se separa y se reúne con el bypass también, lo que aumentará la longitud omitida de los intestinos delgados.

¿Cómo prepararse para la cirugía de revisión?

Los preparativos para la cirugía de revisión son similares a los de otras cirugías bariátricas.

El paciente no debe consumir ningún alimento o líquido 12 horas antes de la cirugía para asegurarse de que el estómago esté vacío antes de la cirugía. Las siguientes restricciones dietéticas y medicinales también deben seguirse antes de la cirugía:

  • El uso de vitamina K debe suspenderse 30 días antes de la cirugía, ya que es un anticoagulante, es decir, diluyente de la sangre, y aumenta la tasa de complicaciones asociadas con el sangrado interno.
  • Los anticonceptivos orales y cualquier otro anticonceptivo que tenga estrógeno o progesterona, como bobinas, parches de TRH o implantes anticonceptivos, deben suspenderse o quitarse 30 días antes de la cirugía, ya que aumentan la tasa de complicaciones derivadas de los coágulos sanguíneos. Los anticonceptivos hormonales y orales se pueden usar 30 días después de la cirugía. Los implantes no hormonales son seguros y se recomiendan si se considera un mini bypass gástrico.
  • Se debe evitar fumar a partir de una semana antes de la cirugía hasta tres semanas después de la cirugía. Fumar hace que las venas se contraigan y limita la circulación sanguínea, lo que puede causar complicaciones como fugas.
  • El consumo de alcohol debe evitarse preferiblemente en los últimos 3 meses previos a la cirugía. El consumo de alcohol una semana antes de la cirugía debe evitarse especialmente, ya que aumentará la carga en el hígado y puede causar complicaciones con la anestesia.
  • El paciente tendrá una consulta con un especialista en enfermedades internas, radiólogo, anestesiólogo y cirujano general antes de la cirugía.
  • Las pruebas preoperatorias en el hospital incluirán ultrasonido abdominal completo, radiografía de tórax, prueba de funcionamiento del aliento, electrocardiograma, investigación endoscópica del estómago y análisis de sangre completo como prueba y exámenes previos a la operación.

When should Revision Surgery be applied?

Revision surgery may be needed if the patient cannot achieve adequate weight loss, or if the patient has regained weight as a result of the operation not being performed optimally.

The first surgery can be transformed into almost any other Bariatric Surgery with revision surgery.

Do diseases such as diabetes, asthma, and blood pressure prevent revision surgery?

Patients with obesity related illnesses are evaluated by a Bariatric Surgeon and decide whether the patient is suitable or not, and similar to other Bariatric Surgeries, Revisional Bariatric Surgery will alleviate these conditions.

Will there be weight gain again after revision surgery?

Revision Surgeries are successful for most patients with regards to permanent weight loss. However, in order to achieve this success, the patient needs to make lifestyle adjustments and change their diet. It should also be known that there is no guarantee of weight loss if these are not done.

Can you have a revision after a Gastric Bypass?

If the pouch between the stomach and the intestine is found to be widened, the “passageway” can be narrowed again endoscopically during the revision operation. The revisional operation also includes the separation of the intestinal loop and it being reunited with the bypass, which will shorten the effective digestive tract.

I had a breast uplift with implant through BookingSurgery.Com on 3rd June 2024, and my partner had dental work done by them too whilst we were there. My partner and I used this company through personal recommendation from someone he knew who had used them for weight loss surgery and then for plastics/reconstructive surgery after. After hearing "horror stories" about surgeries abroad, it was important to get it right. I initially contacted BookingSurgery.Com through their website and a coordinator named Robert Campbell followed up my enquiry through WhatsApp message. After discussion about breast augmentation, Robert consulted with the surgery team and came back to me with a recommendations and quote from two surgeons. The recommendation was breast uplift with implant. I was very happy that the quote included transfers from airport to hospital, the surgery and medications, three meals a day for myself and a companion and their overnight stay too. To secure the price I only had to pay a £100 deposit and the outstanding balance didn't need to be paid until my surgery date. Robert asked me when I'd like to come for my surgery, I gave him an ideal date and he secured it. Robert was a brilliant coordinator, very polite and responded to my queries in good time, liaised with the surgeon and informed me accordingly at all times. When it came to the time for us to come to Turkey, the person who met us off the plane (Gurçan) was very friendly, helpful and a good driver. The cab had WiFi we could connect to which was great. (We also had the same driver who took us back to the airport for our return flight.) We arrived at the hospital and were met by a very happy, welcoming friendly translator called Stella who showed us to our room and explained what would happen next (as you have your pre-op on the day of arrival.) The consent form was very transparent on the surgery I was receiving, what to expect and any risks associated with the procedure which meant I could make an informed choice. The hospital we were at was called Yasam and the rooms, corridors etc were very clean and nice. The nurses were friendly and the care received was of a high quality. The cleaners were friendly and polite too. We had our own bathroom and smart tv and were comfortable. My partner's coordinator, Nora, arranged affordable transfers using the BookingSurgery.Com drivers to take him to the dental hospital in the city where he had his prep work and crowns fitted. The following day was surgery day and I met with my surgeon (Dr Nebil) and the anaesthetist. Dr Nebil really listened to what I was hoping to achieve and very informative. Before going down for surgery, the nurse gave me a sedative as general procedure. I was very happy with the results of my surgery and glad that I followed Dr Nebil's advice of implant with the uplift. His work is amazing! The nurses looked after me very well and the Dr visits you every single day to check on your progress and to answer any questions. There's always a translator on site which makes communication no issue. Pain relief and medications are all included in the care during hospital and when you go home. You receive an information pack to go home with which gives you very detailed advice on how to care for yourself following surgery including when you can resume normal activities such as driving, exercise, sunbathing etc. There are fit to fly letters, medication list with instructions on how to use, all tests taken prior to surgery, and after with the results, and a detailed write up on the surgery process, what was performed and how. I have photocopies of the boxes that my implants came in with serial numbers, details etc, and also contact info for a post op coordinator should there be any issues or if my recovery isn't as explained in the letter. This is very reassuring that BookingSurgery.Com are still looking after me once home. There were 4 translators in the hospital who were all helpful but I want to give a special mention to Stella and Oscar who I felt were more like friends. I met some lovely people on my ward who were all with BookingSurgery.Com for various procedures and having them to talk with definitely helped in terms of company and what to expect in regards of recovery etc. Before leaving Yasam hospital, I was very happy that I was able to have a photo taken with my Dr and translators, and am so happy that I chose to use BookingSurgery.Com
